Fields: Registration Page Here is information about the fields (features) on the Registration page of the Set-up feature.
Include	This checkbox selects which registration fields you would like to include in your study.  The Include field default is unchecked, except for the age, gender, ethnicity and race fields which will default to be included (checked).
Field Label	The field labels are a listing of all the available registration fields. If selected to be included, the field label seen on this page will appear to study participants in the format displayed on the registration pages. If you opt to include Custom Fields, you will need to define the field labels per applicable administration language.
Required	This checkbox selects which registration fields which will be required.  If a participant neglects to respond to one of these fields, they will receive an error message detailing the required fields that have been unanswered.  By default, no registration fields are required.   Warning: Participants may not continue with the assessment until the required fields have been answered.
Validation: Min Value	This field is used to set the minimum value accepted in the corresponding registration field. If a participant enters an invalid response, they will receive an error message detailing the error.    Warning: Participants may not continue with the assessment until a valid response has been provided.
Validation: Max Value	This field is used to set the maximum value accepted in the corresponding registration field. If a participant enters and invalid response, they will receive an error message detailing the error.    Warning: Participants may not continue with the assessment until a valid response has been provided.
PHI	This field is used to indicate the corresponding field may contain Protected Health Information (PHI). When a data field is marked as containing PHI, all data collected in this field will be flagged in the database and will be reported as containing PHI in applicable data exports. This feature is intended to aid in de-identification efforts and protect participant privacy.
Custom Date Field	This field is used to create a custom date registration question.  To include this field, check the Include checkbox and type the question content into the Label for Date Field text box. Expand the applicable language section to view the Label for Date Field text box. A label must be created per administration language.
Custom Number Fields	This field is used to create a custom registration question requiring a numerical response.  To include this field, check the Include checkbox and type the question content into the Label for Number Field text box . You have the option to include two custom number registration questions. Expand the applicable language section to view the Label for Number Field text box. A label must be created per administration language.
Custom Text Field	This field is used to create a custom registration question requiring an alpha-numeric response.  To include this field, check the Include checkbox and type the question content into the Label for Text Field text box. Expand the applicable language section to view the Label for Text Field text box. A label must be created per administration language.
Custom List Field	This field is used to create a custom registration question in which response options are contained in a drop list.   To include this field, check the Include checkbox and type the question content into the Label for List Field text box. Expand the applicable language section to view the Label for List Field text box. A label must be created per administration language. If you opt to include the custom list field, you will need to add the corresponding response options to the validation field to the right.
Validation for List Field	This field is used to enter drop list response options that will be included with the corresponding custom list registration field. Expand the applicable language section to view the List Reponses text box. A list of responses must be created per administration language. Use a comma to distinguish between response options, e.g. lung

Cancel	This button undoes your work since the last save.
