Fields: Consent Page fields

Here is information about the fields (features) on the Consent page of the Set-up feature.

 

Form Section Title bars

Click on the plus (+) sign to begin defining consent form details.

If you do not plan to use some of the available consent forms, simply ignore these sections. 

 

Consent contains Protected Health Information (PHI)

Check this box to indicate the endorsement text entry field may contain Protected Health Information (PHI). This box may be checked for each applicable consent form. 

When a data field is marked as containing PHI, all data collected in this field will be flagged in the database and will be reported as containing PHI in applicable data exports. This feature is intended to aid in de-identification efforts and protect participant privacy.

 

 

Include an Endorsement Checkbox

For each consent form you would like to include in your study, you will be asked determine if participants need to endorse their consent to participate in the study by checking a box.

If you check this box, you will then need to add text in endorsement checkbox text field within each consent language section.  This text will appear next to the endorsement checkbox at the bottom of the online consent document.  The default text is I Accept.

 

Include an Endorsement Text Entry

For each consent form you would like to include in your study, you will be asked determine if participants need to endorse their consent to participate in the study by typing in text (usually a participant’s name).

If you check this box, you will then need to add text in endorsement text entry text field within each consent language section.  This text will appear next to the endorsement text entry field at the bottom of the online consent document.  The default text is Please type in your name.

You can select the Endorsement Text Entry box by itself, or combine it with the Endorsement Checkbox (see above).  You may also opt to not include an endorsement.

  Warning: If you have required an endorsement checkbox or text entry field, but the participant does not enter information or check the box, he/she will not have access to the study.

 

Consent Box

Use this box to copy and paste your existing consent documents, e.g. Consent, HIPAA, or Assent forms.

Basic text editing features are included. However, the formatting of your original form should be retained when you cut and paste it here.

In order to use full word-processing features to change the look of your consent form, you should format your original forms in a word processing program (such as Microsoft Word), then cut and paste it into the consent form content box.
