Fields: Contact Information Page Here is information about the fields (features) on Contact Information page on the Administration tab.
Last Name	This field should be used to record a participant’s last name. NOTE: This is an optional field.
First Name	This field should be used to record a participant’s first name. NOTE: This is an optional field.
Street	This field is used to indicate the street address of a participant. NOTE: This is an optional field.
City	This field is used to indicate the city in which a participant resides. NOTE: This is an optional field.
State	This field is used to indicate the state in which a participant resides. NOTE: This is an optional field.
Zip	This field is used to indicate a participant’s zip code. NOTE: This is an optional field.
Email	This field is used to indicate a participant’s email address. NOTE: This is an optional field.
Day Ph. #	This field is used to indicate the telephone number at which a participant may be contacted during the day. NOTE: This is an optional field.
Cell Ph. #	This field is used to indicate a participant’s cellular telephone number. NOTE: This is an optional field.
Call Time	This field is used to indicate the best time to contact a participant. NOTE: This is an optional field.
Comments	This field allows researchers to add additional information about a certain participant. This is an open text field.  Information that is pertinent to the participant’s contact information should be entered here, e.g. please do not leave message. NOTE: This is an optional field.
