Fields: Consent Data Here is information about the fields in the Consent Data Export.
Participant	Unique ID participants use to access the system.
Form	Consent document included, i.e. Consent 1. If multiple consent forms are included in one study, a participant will have a row listed in the export for each consent.
WasEndorsedChecked	Indicates whether participant endorsed the endorsement checkbox for Consent form .  A value of 1 indicates endorsement was provided. If an endorsement checkbox for Consent 1 was not included the value in this field will be, Not applicable. A participant cannot navigate past the consent form pages if they have not endorsed all applicable fields. Therefore, if a participant exits the data collection pages without completing all required endorsements, they will not be included in the Consent Data export.
EndorsementText	Text entered by a participant in the endorsement text box, usually their name.  If endorsement text was not included this field will read, Not applicable.
AcceptDateTime	Timestamp indicating when applicable endorsement was provided.
Locale	Localization value (Tag for the Identification of Languages) for a consent. Codes for currently available languages are as follows: ·      en-US = US English ·      es = Spanish
