Fields: My Account Page Here is information about the fields on My Account.
User ID	The ID you use each time you log in. A combination of letters and numbers are allowed. This field must be unique within Assessment Center. NOTE: This is a required field.
Password	A password of at least 6 letters, numbers, or a combination of the two. NOTE: This is a required field.
Re-Enter Password	Password verification field. NOTE: This is a required field.
Title	Title or professional designation such Mr., Mrs., Dr., etc.
First Name	First name. NOTE: This is a required field.
Last Name	Last name. NOTE: This is a required field.
Institution	The name of your research facility, university, organization, or other institution. NOTE: This is a required field. The combination of first name, last name and Institution must be unique across Assessment Center.
Email	The email address you would like to use for Assessment Center. NOTE: This is a required field.
Phone	Professional phone number.
Mailing Address	Professional mailing address.
City	Corresponding city name.
State/Province	State (or Canadian province) abbreviation drop list.
Postal Code	Zip code.
Save	If you have  updated any account fields, click this button to process your changes. Assessment Center will send a confirmation email once your updates are saved.
Cancel	The Cancel button does not save any changes you have made.
