Here is information about the fields in the Assessment Data Export.
 
PIN
Unique ID participants use to access the system.
Stcode
Additional unique ID that automatically increments per study starting at 6000 unless otherwise assigned by the study team.
Assmnt
Data collection time point (e.g. assessment 1, assessment 2);.
MdlOrdr
Grouping field within data collection time points (Assmnt). Can be used to randomize instruments or group of instruments. Instruments with the same MdlOrdr value were administered together in the same module.

Multiple instruments can be grouped together (e.g., all demographic instruments together).  These groups are called Modules.
 
InstrOrdr
Administration position (1st, 2nd, etc.) of an Instrument within a Module (MdlOrdr).
InstrSctn
Grouping field within an instrument which can be used to present items together during administration. Items with the same InstruSctn value were administered together in the same block.

Within one instrument, you can group items together (e.g., that share response options).  These groups are Item Blocks.
 
ItmOrdr
The position of an item within an item block (InstSctn).  This may or may not match the order of administration based on how a study was set-up.
Instr
 Instrument name as entered on the Instrument Properties page.
Locale
Localization value (Tag for the Identification of Languages) for an instrument. Codes for currently available languages are as follows:
·      en-US = US English
·      es = Spanish
Mode
Mode of Administration (i.e. Participant via Web, Proxy via Web, Participant via IVR).
ItemID
ID of the item administered.
 
PHI
Indication of whether field may contain Protected Health Information (PHI)

True = Consent contains Protected Health Information (PHI) checkbox was endorsed on Consent Forms page.
False= Consent contains Protected Health Information (PHI) checkbox was not endorsed on Consent Forms page.
Rspnse
Position within response options of participant selection.  For example, the response option listed first is coded as 1.

If a participant skipped an item, the Rspnse field will be blank. 
Score   
Score of the participant selection.  For example, the response option listed first could have a score of 5 (5=not at all).

If you use a checklist response option, a bitwise value is listed.  A checklist allows a participant to check more than one response.  Each response is assigned a mathematical value.  All responses are summed.  The total is listed in the assessment data file.  The response option scores are such that one can reach a sum through only one combination of responses.  The bitwise scores are 1, 2, 4, 8, 16, 32, 64, etc.  Therefore, if a participant has a score of 9, you know that he or she endorsed the first (1) and fourth (8) responses.  If a participant has a score of 16, you know he or she endorsed just the fifth response choice.

If a participant skipped an item, the Score field will contain the word SKIP.  Participants are currently not able to skip items in PROMIS Profile instruments.

 
Theta
Instrument score.

The theta field will only be populated in the Assessment Data Export for CAT instruments. To find the thetas for PROMIS Short Forms access the Assessment Scores Export.
T-score
Transformed score based on a (10*Theta +50) formula.

The Tscore field will only be populated in the Assessment Data Export for CAT instruments. To find the Tscores for PROMIS Short Forms access the Assessment Scores Export.
SE
Standard Error for administered instrument.

The SE field will only be populated in the Assessment Data Export for CAT instruments. To find the SE for PROMIS Short Forms access the Assessment Scores Export.
DataType
Response type (e.g. integer, string).
Postn
Administration order of item.  If randomization is used, this will be different from ItmOrdr.
Time
Time, in seconds, between presentation of item and user response.
DteCrted
System date & time stamp of response.

InstrStr
System date & time stamp when a participant starts an instrument.

System date & time stamp when a participant finishes an instrument.

Consent
Study Consent (1=yes, 2=no, 3=test).
